Patients can conveniently schedule a telehealth appointment through several easy methods:
Simply call either of our locations to request a telehealth visit.
Use the contact form on our website to submit a telehealth appointment request.
Patients may self-schedule same-day or future appointments up to five days in advance. When booking, please enter “Televisit – [reason for visit]” in the Reason for Visit field.
Use the Phreesia online scheduling portal to request a telehealth visit. Be sure to include “telemed visit” along with a brief description of the acute or chronic issue in the Reason for Visit section.

On the day of your appointment, a text message containing the secure visit link will be sent to the phone number we have on file. If you do not receive this link prior to your appointment, please contact the office by phone or text for prompt assistance.

Patients seen by Dr Azimi may receive telemedicine care in any of these states, as Dr Azimi is licensed to practice in each. If you are outside these states and would like to schedule a telemedicine appointment, please confirm your current location when booking.
Controlled Substances Policy:
Please note, for urgent care telemedicine visits, we do not prescribe controlled substances under any circumstances.

A credit card is needed for a telemedicine visit. You may use insurance that is accepted by MI Express Care or have a Self-Pay Visit for special introductory promotional offer of $85 per visit. If you use insurance, verification of eligibility and urgent care benefits will be performed.
Guarantor agrees to pay applicable urgent care copays and/or deductibles at the time of service. Guarantor is responsible for any amounts not covered by insurance. If needed, your provider can e-prescribe a prescription to your pharmacy.
