Shoulder pain shows up differently depending on what is going on underneath, but a few patterns are common enough to watch for.
Most shoulder pain traces back to the rotator cuff, the group of muscles and tendons that hold the shoulder joint together and allow the range of motion the joint is known for.

Tendonitis is inflammation and irritation of the rotator cuff tendon, usually from overuse, and it tends to respond well to non-surgical treatment. A partial or full tear is a structural break in the tendon itself, and while some tears also respond to non-surgical care, others need a surgical opinion. The two can feel similar day to day, both can cause pain with overhead movement and discomfort at night, which is exactly why guessing from symptoms alone is not a great plan.
